I. PILES:- Piles can be internal & external & initially can be characterized by slight pain & itching sensation while defecation,later on it becomes worse with daily bleeding & pain along with external or internal lumps or lobules like structures.
Piles or Hemorrhoids are broadly classified as
1. Internal Piles (Hemorrhoids) : Occurs in Anal Canal & Rectum.
2. External Piles (Hemorrhoids) : Outside Anal Canal near anal opening.
Grade-I :- Hemorrhoids (Piles) are still in the interior anal canal & there is no protrusion or prolapsed. However there is minor itching or pain.
Grade-II :- Hemorrhoids (Piles) are similar to grade-I however bleeding is also observed at this stage. Additionally some small protrusion will come outside during bowels & reverts back to its original position.
Grade-III :- When hemorrhoids reaches at stage-III, they have all the symptoms of earlier stages only the prolapsed or protrusion will not return to its original position on its own & must be manipulated back into the anal canal manually.
Grade-IV :-This is a final stage and haves all symptoms of earlier stage except haemorrhoids can no longer be manipulated back into place in the rectum.
III. FISTULA :- Fistula-in ano can be characterized by a formation of small opening near anus,through which a pus & watery discharge comes out. It can simply look like a "boil" which forms & bursts in certain time intervals. Fistula can be a single or multiple tract also.
II. FISSURES:- Fissures are characterized by extreme Pain , Burning sensation & needle pricking sensation during defecation & in extreme cases through out the day also.Sometimes it shows little bleeding also.

In all Ano-rectal diseases major contribution is of a Varicose veins inflammation which in normal terms are swollen veins. Increased straining and pressure in the veins of anus and rectum causes hemorrhoids. This condition is often a result of constipation when we strain during defecation.
There are many ways of treatment of ano-rectal problems. Internal hemorrhoids can be treated by banding which is usually not a painful process. This is done by placing a tight rubber-band around the base of hemorrhoids. Large external or internal hemorrhoids are treated under a local or general anesthesia & veins are cut out.
Some hemorrhoids can be treated by a injecting a special material, which shrinks them and stops bleeding this is called “Sclerotherapy”.
Hemorrhoids can also be burned or coagulated with techniques like lasers & Infra-red treatment. Unfortunately none of the above methods guarantees that hemorrhoid will not relapse. Therefore it is very important to change your lifestyle & eating habits.

This is very common but not limited to anal fissure scissor, as it generally involves “lateral Sphincterotomy” involves making a small cut in sphincter muscle to reduce the tension in anal canal.
☛ Repeated scissor techniques in Fistula patients can create a bigger wound, which can be a difficult affair to be close by any other technique.
