Piles or hemorrhoids are clumps of dilated (enlarged) blood vessels in the anus and lower rectum. The rectum is the last area of the large intestine before it exits to the anus. The anus is the end of the digestive tract where feces leave the body. Or Whenever there is inflammation in hemorrhoidal tissue (hemorrhoids are a cushion of tissue i.e. full of blood vessels and elastic fibers), it causes a problem of piles Mostly Piles forms when the veins enlarge and their walls become stretched, thin, and irritated by passing stool due to constipation.
Internal hemorrhoids / Internal piles : are found inside the rectum. They cannot be seen or feel from outside as they are located deep inside the anus. when these hemorrhoid becomes large can cause bleeding and pain as there is a friction between piles and faeces or waste material.
Grade-I :- In this grade of piles / Hemorrhoids are still in the interior anal canal it is the initial stage of piles having slight itching and pain.
Grade-II :- Morphologically they are similar to gradeI,but patient experience sometime bleeding Additionally some small protrusion will come outside during bowels & reverts back to its original position.
Grade-III :- At the stage-III, they have all the discomfort of earlier stages additionally having prolapsed or outgrowth that will not return to its original position on its own & must be manipulated back into the anal canal manually.
Grade-IV :- This is a final stage and haves all symptoms of earlier stage except haemorrhoids can no longer be manipulated back into place in the rectum.
Prolapsed piles : sometimes internal hemorrhoids can swell and comes out of your anus. This is also known as a prolapsed piles. These are swollen red coloured causes pain, itching, irritation or burning and sometime bleeding also.
External piles : External hemorrhoids present on the mouth of anus, or on the surface from where faeces come out. And usually seen as lumps on the anal surface. Symptons of external and internal piles are more or less same but one can feel more discomfort to sit, doing physical activities or while passing stools.
Thrombosed piles / thrombosed hemorrhoid : is having blood clot (thrombosis) within the haemorrhoidal tissue. It is usually outside the anus in swelled up form. Such kind of piles are having lot of pain, swelling, redness, itching and usually bluish red in colour in short complicated case of piles.
Ksharsutra made up of two words "kshar(alkali),and sutra(thread) which means treatment with medicated thread. Treatment of anorectal diseases through Ayurveda is becoming very popular in India. At present, treatment of Piles-in-ano with the help of Kshar sutra is very popular among ayurvedic as well as allopathic surgeons. Kshar sutras consist of three steps:
(1) Dissolution of drug.
Ksharsutra technique is mainly concerned with the coating material of drug by various means of kshar and latex. In this technique different type of coating materialis spread over thread; In some cases coating material is coated in the form of beads over the thread called “BEADED” Kshar-sutra but in some cases two different threads with different coating is used called “DOUBLE” Kshar sutra technique. Generally, kshar sutra causes lot of discomfort to patient. Initially, after diagnosing the case, probing is performed and a kshar sutra or plain cotton thread is passed through the anal cannal. This is called primary threading. This thread is kept in tract for one week; patient has to visits OPD weekly to change it. This procedure is called secondary threading. Secondary threading is necessary, because it is observed that the effect of coated drugs diminishes by time and cotton thread also become so weak, that it may disintegrate itself to shed off. hence medicated thread should be changed timely to get complete relief In case of piles medicated thread (Kshar Sutra) is tied at the base/root of the pile mass so that, the blood supply is cut and pile mass starts to dry. After a few days, pile mass sheds off. and finally the root is permanently damaged through Kshar Sutra thereby reducing the chances of recurrence.
Side effects of ksharsutra
• Kshar Sutra Therapy for Piles is Always Very Painful and Prolonged.
• There is no proper scientific authenticity.
• Pain and complications post-surgery.
• Chances of bleeding, pain and infection could be there.
• Not able to treat grade 4 hemorrhoids.
What are Piles or Hemorrhoids?
☛ Haemorrhoids, also known as piles, are enlarged veins situated in or around the back passage (anus). They occur when these blood vessels become swollen or pathological.
How Piles are caused?
☛ Under pressure the blood vessels in and around the anus stretch & becomes inflamed. Chronic coughing and lots of heavy lifting are common ways that pressure in the abdomen is increased and that in turn results in increased pressure in the veins located around the anus. Pregnancy, being overweight and childbirth can also cause piles.
Will diet & lifestyle improvement solve the symptoms of piles?
☛ As straining on the toilet is one of the common cause for piles to develop, avoiding constipation and having to strain is important to improve symptoms. Eating a healthy diet, having plenty of fresh fruit, vegetables and other dietary fibre, drinking plenty of water, keeping active and keeping stress levels under control & proper sleep will help in regular & proper bowels.
Ayurvedic ingredients like trifla are very common in avoiding constipation.
Are skin tags the same as piles?
☛ No, skin tags aren't the same as piles, but you may have them if you get piles. Usually, you won’t need any treatment for skin tags. Taking care to clean around any skin tags after every bowel movement will reduce soreness and itching
Does sitting on cold surfaces cause Piles?
☛ No. This is a very common belief but it’s a myth. Sitting on hard or cold surfaces may make haemorrhoids feel more uncomfortable for those people who already have them but will not actually cause Piles. Ignoring the urge to go to the toilet, inactivity, missing breakfast or eating on the go, however, can cause constipation, which makes straining and consequently haemorrhoids more likely.
What can I use to reduce the symptoms of Piles?
☛ Ice packs help reduce swelling and sitting in a warm bath filled with plain water for 5-10 minutes three times a day will relieve symptoms too. Try not to sit for long periods of time since this may cause more irritation. You can visit your Doctor for advice on treatment to relieve pain and irritation. Stool softeners can be used to get the bowels working properly again and help to eliminate the need to strain.
What can I do to stop getting Piles again?
☛ Make sure your diet contains plenty of fibre, drink lots of fluids and increase the amount of exercise you get. These measures will help prevent constipation, one of the main causes of Piles.
My job requires full day sitting in front of computer. Will this give me piles?
☛ Sitting for long intervals on the same place increases the ano-rectal complications. Have regular breaks, in these breaks stand up and walk around. At lunchtime, take a walk, even just for minute or five.
What will happened if I do not take treatment for my Piles?
☛ Condition can become worsen with time, eventually leading to complications. Surgery is needed only in about 10% of patients with piles. So we should not delay in getting an early diagnosis & proper treatment. Ayurvedic medicines for piles are one of the best piles treatment options.
